#bitesize – Sophia Nimphius on the Hidden Causes of ACL Injuries Coaches Often Miss

Hidden ACL Injury Factors

Recognizing Hidden ACL Risks

Understanding the hidden ACL injury factors is essential for any coach aiming to protect their athletes. Sophia Nimphius highlights that most ACL injuries happen during moments of deceleration, landing, or side-stepping—especially when the knee is extended. While this is common across many sports, hidden ACL injury factors such as poor body positioning, fatigue, or late-stage game decisions become even more critical in situations like defensive duels in AFL or soccer, where unpredictable movements and reactions increase the risk.

The Fatigue Connection

Fatigue is more than just tired legs it influences how athletes move. When fatigued, athletes tend to shift into more dangerous positions. While variability in movement usually helps distribute load safely, losing this variability under fatigue exposes joints to stress. Especially problematic is when athletes move in ways they haven’t practiced or conditioned for.

Observing Movement in Context

Rather than relying solely on lab-based tests, Nimphius prefers watching athletes during play. This approach provides insight into real-time decisions and movement patterns. By using sport-specific drills with constraints, she determines whether an athlete avoids certain movements due to habit or true physical limitation.

When and Why to Intervene

Importantly, changing movement patterns requires weighing the potential benefits. If a movement doesn’t pose a serious injury risk or improve performance, it might not be worth the time and effort to correct it. Additionally, athlete psychology and motivation play a vital role in long-term change.

The Role of Technology

Although researchers are exploring methods to monitor ACL strain, current technology isn’t there yet. Promising studies using T2 MRI are underway, but nothing is ready for everyday use just yet.
